The Cuyahoga County Board of Health will be administering H1N1 vaccines for the North Royalton students on January 13th and 14th within our six school buildings. The vaccine is NOT mandatory and will be provided free of charge. Students will be bringing home further information concerning the vaccination clinic on Friday, December 18, 2009.
Additional information including the consent form and questionnaire will be sent home with your child on January 4, 2010. If you wish for your child to be vaccinated, please sign and complete both the consent form and the questionnaire and return it to school by Monday, January 11th, 2010. STUDENTS WHO HAVE NOT RETURNED THE FORM WILL NOT BE VACCINATED. North Royalton City Schools are only serving as a vaccination location. The vaccine is being provided and administered by Cuyahoga County Board of Health and therefore all questions concerning the vaccine including side effects should be placed to (216) 201-2000 (option 5).
